Patent Claims:	1. A pharmaceutical dosage form comprising: a) about 200 mg of a solid dispersion comprising about 100 mg of velpatasvir dispersed within a polymer matrix formed by a pharmaceutically acceptable polymer A, wherein the velpatasvir is substantially amorphous; b) about 400 mg of sofosbuvir wherein the sofosbuvir is substantially crystalline; and c) about 200 mg of a solid dispersion comprising about 100 mg of voxilaprevir dispersed within a polymer matrix formed by a pharmaceutically acceptable polymer B, wherein the voxilaprevir is substantially amorphous, wherein the polymer A and polymer B are copovidone. 2. A tablet, comprising: a) about 400 mg of sofosbuvir wherein the sofosbuvir is substantially crystalline; b) about 200 mg of a solid dispersion comprising about 100 mg of velpatasvir dispersed within a polymer matrix formed by a pharmaceutically acceptable polymer A, wherein the velpatasvir is substantially amorphous; and c) about 200 mg of a solid dispersion comprising about 100 mg of voxilaprevir dispersed within a polymer matrix formed by a pharmaceutically acceptable polymer B, wherein the voxilaprevir is substantially amorphous, wherein the polymer A and polymer B are copovidone.
